Collection of information
We understand the confidential nature of the information that you provide us. We collect health and personal information for the purpose of providing treatment to you. This link (insert link) sets out an example of the template we use when we first see you. We will continue to collect relevant information over the course of your treatment.
Use of information
The information that you provide is used in accordance with our obligations under the Privacy Act 1993 and the Health Information Privacy Code 1994 for:
- Your treatment and care
- Liaising with other treatment providers and ACC as required by you
- Monitoring the provision of this treatment
- Providing you with information that you may ask for
- Responding to any queries or concerns that you may have
- Administering our financial records
Storage of information
Your information is stored securely. We are required to keep your information for 10 years after your last consultation with us under the Health (Retention of Health Information) Regulations 1996.
Access to information and correction
You can ask to access your health information by writing to or emailing us. We may need verification of your identity. Chiropractic records contain technical references and so a summary may be provided if a request is made, to assist you. You are entitled to request a correction if you think that information may be incorrect.
